What to do if you move while your case is pending
Below please find a link to Form AR-11, which is to be completed in the event of a change of address. Individuals subject to the Special Registration requirement should submit Form AR-11SR.
It is recommended that Forms AR-11(SR) be mailed via certified mail/return receipt requested to USCIS.
Forms AR-11(SR) need only be filed with USCIS in the event of a change of address.
Medical Forms
Aliens seeking an Adjustment of Status are required to submit Forms I-693 Medical Examination and Form I-693 Vaccination Supplement. Both forms can be accessed online and printed by clicking 
here.
Form I-693 and the Vaccination Supplement may only be completed by a Physician authorized by USCIS. A searchable list of designated Physicians is available here.
